Goals of Passages:
To facilitate supported and informed choices about future life directions
To facilitate the transition from school to work opportunities, community-based activities, and community living to access time-limited supports that strengthen community involvement skills
To participate in programming to build and strengthen identified skills and provide exposure to a wide variety of opportunities
Services Provided:
Information sessions that facilitate choices about future life directions Transition planners that assist with the transition from school to work opportunities, community-based activities, and community living
Time-limited supports that strengthen community involvement skills Partnerships with Halton Catholic and Halton District School Boards that build on and identify skills that will provide exposure to a variety of opportunities Assist with referrals to community activities
